HEART & SOUL CARE MANAGEMENT & CONSULTING provides intensive care management for older adults and for people with chronic medical and psychiatric illness, who require support to live well at home. We offer the necessary support to ensure that medical, social, financial and emotional needs are addressed. Our comprehensive care planning allows families to relax, knowing their loved-one’s long-term care needs are managed by competent professionals. We coordinate services with health care professionals to provide continuity of care for their patients or clients. We also assist Hospital Case Managers & Discharge Planners to meet their length of stay requirements and safe discharge planning for complex cases.


EVALUATION FOR HOME SAFETY & FUNCTIONAL ABILITIES ASSESSMENT:
